- I hereby declare that I am an authorized user of Graceland University’s Fleet Services; I also declare that I have no traffic violations which would disqualify me as an authorized driver since my last Motor Vehicle Record (MVR) check.
- I acknowledge that I am responsible for any traffic tickets received while operating a University Vehicle.
- I will remain in compliance with the University’s policy regarding drugs, alcohol and tobacco use while using this vehicle.
- I accept responsibility for the credit card issued in the vehicle packet. CREDIT CARD PIN #1234 The credit card is for vehicle expenses only.
- DO NOT USE FOR PERSONAL ITEMS! Other usage will be charged to the department responsible for the vehicle and may result in additional service fees. Vehicles are to be returned full of fuel.
- Cell phone use permitted only with a hands-free device while the vehicle is in motion. Texting is strictly prohibited while driving.
- All passengers must wear seat belts and all traffic laws must be obeyed. All drivers of this vehicle must be properly authorized.
- A $100.00 service fee, per incident, will be charged to the department if the following are not met:
- Trash must be removed from vehicle.
- Damages to vehicle must be reported on the bottom of this form.
- Vehicle must be returned, filled with fuel, on the date and time specified on this form. Please contact Facility Services at 641.784.5191 if you need to extend your reservation.
- Vehicle packets can be dropped off in the drop boxes located in each parking lot after hours, or in the Facility Services Office during regular business hours.
- No animals are permitted in Graceland University vehicles.
Vehicle Mileage Rates: Car, SUV, or Minivans-$.73 per mile, 12 Passenger Vans- $.88 per mile, 15 Passenger Vans-$1.25 per mile.
