Cooperating (Classroom) Teacher Information

The cooperating teacher is the team member with whom the student teacher works most closely. You play a vital role in the professional growth of the student teacher, and will help to further the student teacher's confidence, skill, and competence toward performance in the classroom.

Forms and instructions are provided for your convenience below in both Word and PDF formats. Click on the link to open or save the document to your computer. To complete a form electronically, save the document (Word format) to your computer and open it with Microsoft Word. The PDF forms can be opened and printed from your web browser or Adobe Acrobat, then completed by hand. (Refer to Troubleshooting Tips below if you need help.) The University Supervisor will collect your completed forms during their observation visits.

Program Evaluation
We request that you complete a Program Evaluation survey at the end of the student teaching period that will help us evaluate and improve the Teacher Education program. Sometime during the final weeks of the student teaching period you will receive an email which includes a link to the online survey. We appreciate your willingness to complete this quick survey and give us honest answers about your experience with the Teacher Education program.

For a Seven-Week Placement:
Lesson Critique (2) - during weeks 1-3 and weeks 4-7
Evaluation (2) - end of 3rd week and end of 7th week

For a Fourteen-Week Placement:
Lesson Critique (4) - during weeks 1 - 3, weeks 4-7, weeks 7-9, and weeks 10-14
Evaluation (4) - end of 3rd week, 7th week, 9th week, and 14th week

All forms will be collected by the University Supervisor during observation visits.


Student Teacher Requirements

  • Document 70 days of full-time student teaching on a time sheet signed by the cooperating teacher
  • Create plans for each lesson taught
  • Observe, participate, and assume full-time teaching responsibilities
  • Adhere to school regulations and student teaching expectations
  • Submit weekly journals in ePortfolio
  • Submit Required Artifacts in ePortfolio (at acceptable or above)
  • Submit a Disposition Self-Evaluation at the end of 7 weeks or first placement
  • Be formally evaluated by the cooperating teacher (at acceptable or above)
  • Be formally evaluated by the university supervisor (at acceptable or above)
  • Submit a Self-Evaluation form after each student teaching placement
  • Complete a Program Evaluation
  • Complete a post demographic survey (ePortfolio)
  • Participate in an exit interview with the university supervisor

To Use Electronic Word Forms (Windows)
1. Click on the form (Word format) and choose "Save" to save the file to your computer. (If you don't get this option, refer to Computer Settings below)
2. Open the form with Microsoft Word, complete it electronically, and save your changes.
3. Print the form and give it to the University Supervisor during their scheduled visit.

Computer Settings (Windows)
If you don't get the option to Open or Save a form after clicking on it once, follow these directions to change your computer settings.
1. Open My Documents
2. Go to Tools>Folder Options
3. Go to the File Types tab and select "(.DOC) Microsoft Word Document" in the list of file types.
4. Click the Advanced button and put a checkmark beside "Confirm open after download".
5. Click OK

To Use Electronic Word Forms (Macintosh)
1. First, determine the location of downloaded files
a. Open your web browser and go to File>Preferences
b. Under "Save downloaded file to", choose Desktop
2. Download forms
a. Click on the form
b. The web browser will automatically download the file to your Desktop and the Download Window will open. To open the file, either double-click it in the Download Window or go to the Desktop and open it there.

PDF Forms
Forms in PDF format will open automatically within the web browser, allowing you to either view or print.
