The Institutional Review Board
With this in mind, the university has created an Institutional Review Board (IRB) for the purpose of assuring that the rights of human subjects are protected while participating in research sponsored by the institution. Members of the IRB are charged with evaluating the likelihood of negative impact and/or assessing the risk/benefit ratio of research which involves the participation of human subjects.
Before beginning research, an application must be submitted to the Institutional Review Board. Please follow the steps below:

1. Click on the Registration form below, save it to your computer, and then open with Microsoft Word or another word processor. Complete all information requested on the form.

3. Write a brief description of the proposed study that includes:
4. As applicable, attach copies of instruments (e.g., surveys, interview outlines), consent documentation (e.g., cover letter, consent form), debriefing statement, and letter of agreement from cooperating institution(s). A sample Informed Consent document is available below.
5. Above documentation should be submitted to your course instructor as email attachments.
