Complete this Application for Admission Form in detail. Indicate "N/A" if an item is not applicable. Items you feel may be discriminatory are optional.

Call the Student Information Office (800-833-0524) to receive prices for Application Fee, Tuition, Resource Materials Fee and to give your Visa or MasterCard information.

Transcripts:The University is required to collect an official high school transcript or GED unless you have an associate or bachelor's degree. After submitting your application, you must also submit official transcripts from each college and/or school of nursing which you attended, even if credits from more than one institution are shown on a particular transcript. Official transcripts must bear the seal of the institution and be sent directly to Graceland University by the sending institution.

BSN Applicants: Reference forms will be provided by the University upon receipt of the completed Application For Admission. One reference form should be submitted by a previous instructor, and the other by your current or most recent employer/supervisor. If a previous instructor is not available, both forms may be submitted by employers/supervisors.

BA-Health Care Administration Applicants: Guidelines and intructions for requesting credit for continuing education programs and professional nursing experience will be provided by the University upon receipt of the completed Application for Admission.
