University Supervisor Information
University supervisors are an important liaison between the school/cooperating teacher and the School of Education. Creating positive relationships with both the cooperating and student teacher will improve communication and help make this a successful experience for everyone involved.

Student Teacher Requirements
- Document 70 days of full-time student teaching on a time sheet signed by the cooperating teacher
- Create plans for each lesson taught
- Observe, participate, and assume full-time teaching responsibilities
- Adhere to school regulations and student teaching expectations
- Post weekly journal entries in ePortfolio to School of Education Supervisor
- Submit Required Artifacts in ePortfolio
- Submit the Disposition Self-Evaluation at the end of 7 weeks or first placement
- Be formally evaluated by the cooperating teacher (at acceptable or above)
- Be formally evaluated by the university supervisor (at acceptable or above)
- Submit a Self-Evaluation form after each student teaching placement
- Complete a Program Evaluation
- Complete a post demographic survey (ePortfolio)
- Participate in an exit interview with the university supervisor

To Use Electronic Word Forms (Windows)
1. Click on the form (Word format) and choose "Save" to save the file to your computer. (If you don't get this option, refer to Computer Settings below)
2. Open the form with Microsoft Word, complete it electronically, and save your changes.
3. Print the form and give it to the University Supervisor during their scheduled visit.
Computer Settings (Windows)
If you don't get the option to Open or Save a form after clicking on it once, follow these directions to change your computer settings.
1. Open My Documents
2. Go to Tools>Folder Options
3. Go to the File Types tab and select "(.DOC) Microsoft Word Document" in the list of file types.
4. Click the Advanced button and put a checkmark beside "Confirm open after download".
5. Click OK
To Use Electronic Word Forms (Macintosh)
1. First, determine the location of downloaded files
a. Open your web browser and go to File>Preferences
b. Under "Save downloaded file to", choose Desktop
2. Download forms
a. Click on the form
b. The web browser will automatically download the file to your Desktop and the Download Window will open. To open the file, either double-click it in the Download Window or go to the Desktop and open it there.
PDF Forms
Forms in PDF format will open automatically within the web browser, allowing you to either view or print.
