Here are some forms that help ITS serve the campus community. They are to request technology related equipment/software, access to Graceland's telephone and computer services, and office moves. Using these forms will help our operations run smoothly. Please be sure to call the Help Desk at 5167 if you have any questions about any of these forms.

To purchase technology related equipment (including software) or a telephone- use this form for these types of purchase requests. You will be contacted after pricing has been confirmed.

**In most cases a budget revision is required, unless the complete purchase is covered by grant or gift funds. Purchase Orders are held until the budget revision approval is received from Accounting.

A new hire in your area will need computer access or telephone services- this form will insure these needs are met.

Lamoni and Independence

If rearranging or moving to another office that entails moving the phone or computer to a different area- this form will insure a smooth transition.
