All full time employees are eligible for the following benefits. Part time employees are eligible for health and dental insurance without a Graceland subsidy. Benefits begin the month after hire date. Premiums are deducted the month before coverage period.
Health Insurance
Principal Insurance Company is the carrier, PPO plan with 3 different deductible options, premiums are based on deductible chosen. Graceland pays roughly 70% of the premium. Graceland implemented a High Deductible Health Plan and Health Savings Account option on January 1, 2006. Please use the link in the right sidebar to visit their website. If enrolled, you will be able to get claims and benefits information by logging in with your Principal ID number.
Delta Dental Insurance
Delta Dental is the insurance carrier. Graceland pays 50% of the premium. Preventative services are free twice a year. All other covered services are paid at 50% by the insurance plan and 50% by the employee. A link to the Delta Dental website is provided on the right side of this page.
Vision Insurance
Avesis is the carrier. Voluntary vision insurance is available to employees. A basic eye exam is just a $10 copay. Lenses and frames are a $25 copay. Contact lenses no copay. Allowances vary. Please make sure that you are using an Avesis participating provider. A link to their website is provided on the right side of this page.Life Insurance
Sun Life is the carrier. Graceland pays for life insurance equal to the employee's annual salary up to a maximum of $50,000. Graceland also provides $2,000 coverage on spouse and dependents. Supplemental life insurance is available for employee purchase.Accidental Death & Dismemberment Insurance
Sun Life is the carrier. Graceland pays for coverage on the employee equal to their annual salary. Provides coverage 24 hours a day in the event of an accident. Optional coverage is available for purchase by the employee.Long Term Disability
Graceland provides long term disability coverage on all full time employees. This insurance will replace 60% of wages in the event that the employee becomes disabled and unable to work for longer than 90 days.Flexible Spending Account
