Benefits Forms Due by May 14

Please remember to turn in your benefit change forms to Human Resources by 5 p.m. on May 14.  All benefits are deducted the month prior to coverage, so changes will be made on your May 31 payroll.  All changes will be effective June 1, 2008.  If you have questions or need forms, please call HR at ext. 5174.  Thank you for your cooperation.
